The French health care system, under their version of national insurance, is excellent. Private health insurance is usually just taken out as a top up over here. Private insurance is not too expensive either - relatively speaking. In terms of pay conditions French nurses, doctors, dentists and all related health care occupations are reasonably well paid. Another big difference is that in France it is not possible to pursue frivolous law suits and there are no ambulance chasing lawyers. There fore insurance costs are kept down.

Yes, it really is better and, as a percentage of GDP, it costs less than the American system. This system works for a variety of reasons. To begin with, higher education in France is all but free and therefore doctors and nurses don't begin their professional lives with a crushing burden of debt. Next, malpractice lawsuits are less frequent and don't produce huge judgments. But the principle reason is that health care workers make vastly less that they do in the United States. The average annual compensation for a physician in France is between 65 and 125 thousand euros, a tiny fraction of what an American physician makes. Salaries for nurses and other health care workers are also much lower than in the USA. This means that fees are lower even before insurance reimbursement is taken into account with office visits costing 21 euros ($30) and doctors making house calls for 31 euros (i.e. about $45). Medicine is not as industrialized as it is in the USA and I suspect that this is one reason why it is so effective in France. By most standard measurements, the French have the best health care system in the world.

France's health insurance is definitely a lot cheaper than in the US, but I still find the quality of care to be MUCH better in the US. I also find French doctors to be not very advanced technologically speaking. (For example, my eye doctor here uses a technique that my eye doctor in the US stopped using years ago to replace with a more modern and faster technique.) For nurses, the salaries are decent and conditions are good. However, don't make any plans to move over here and become a nurse. You'd need to attend French nursing school and pass the French nursing exam to do so.
